What supports health in the community?

Imagine Southwark wants to build a healthy community where:
People are involved in process that inspires them to think and act constructively about the ways they can influence their own health, the care they receive and the health of their communities.

We can do this by finding out:

What people believe constitutes good health and when they felt healthiest
What people believe constitutes good care and where they received that care
When and how people are making important decisions about their health and care (or the health and care of someone else).
Conditions that enable people to "take charge" of their own health and care
What we can do to improve the health of our communities

By studying the responses from the public AND Health professionals, we can

Reveal the patterns about the ways that people perceive health and care
Discover practices that already exist that are helping to make people healthier, providing them with the care they want, and enabling them to make important health-related decisions
Uncover new ideas for improving health and care
